What You'll Do:
Program Leadership:
Develop and manage comprehensive chronic care case management programs tailored to high-risk patient needs.
Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to create and implement effective patient care plans.
Continuously monitor program outcomes, analyze data, and make necessary adjustments to optimize patient care and program efficiency.
Care Plan Implementation and Adherence:
Develop and deliver training to staff on creating and maintaining high-quality care plans and documentation standards.
Ensure adherence to evidence-based practices and quality standards, advocating for preventive care and early intervention strategies.
Ensure proper and timely documentation.
Population Health Strategies:
Analyze patient data to identify trends, opportunities for improvement, and potential disparities in care.
Drive initiatives that enhance patient outcomes, reduce healthcare disparities, and improve overall population health.
Team Leadership and Mentorship:
Lead, mentor, and support a team of RN care managers and healthcare professionals.
Foster a collaborative and supportive work environment that encourages professional growth and development.
Delegate effectively and motivate team members.
Quality Assurance and Compliance:
Ensure compliance with all relevant regulatory requirements and industry standards.
Monitor program quality, identify areas for improvement, and participate in quality improvement initiatives.
What You Bring:
Education and Experience:
Bachelor's Degree in Nursing with a minimum of 5 years of RN experience in care coordination, case/care management, utilization/resource management, and/or transition of care. OR
Associate's Degree in Nursing with a minimum of 7 years of RN experience in the same areas.
A minimum of 2 years of supervisory nursing experience is required.
Licensure and Certification:
Active Registered Nurse (RN) license in Washington State or a valid Multi-State RN license.
Necessary compliance with Washington state license requirements as described in the original job description.
Case Management Certification (CCM) is highly valued.
Skills and Abilities:
Strong knowledge of utilization management, quality improvement, discharge planning, and cost management.
Understanding of medication uses and effects.
Cultural sensitivity and the ability to work effectively with diverse populations.
Excellent verbal, written, and listening communication skills.
Proven leadership and team management abilities.
Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint).
Bilingual in English and Spanish is valued.

Competitive salary range: $89,000 - 135,000(dependingonexperience).∗GenerousSignonbonus,with7,500 on the first paycheck, and 2,000$ at 12 months.
