Job Description
We are looking for an LPN/RN to be our Director of Patient Care. You will be responsible for managing two nurses and one social worker. This individual will work collaboratively to promote quality care and cost-effective outcomes that will enhance the physical, psychosocial, and vocational health of our patient population. This position is mostly remote, working from the office, very often from home and very often in the field. The DPC will be required to work in the Wesley Chapel office at least once per week. Responsibilities:
Lead the patient care team (two nurses and one social worker)
Develop and administer policies and procedures for utilization monitoring and management
Adhere to Policies and Procedures
Apply appropriate data collection techniques and tools
Maintain and improve productivity and patient care operations by monitoring team and system performance to identify and resolve problems
Mentor and guide team
Maintain good community relationships
Follow all Federal, state and local laws, licensure/certifications in an ethical manner
Collaborate and communicate with other managers and other departments within the company other duties, as assigned
Qualifications:
At least three years as a Director of Utilization Management, Utilization Review, Care Management, Care Coordination or Case Management
Certification in appropriate field of expertise (ie: Certified Case Manager, Chronic Care Professional) preferred
Must be n LPN or RN with active and unrestricted license to practice in Florida
Ability to attend to numerous details under the stress of maintaining courteous, accurate and timely relations with a variety of individuals
Ability to work independently, with minimal supervisor
Ability to comprehend the consequences of various problem situations and to refer such problems to the appropriate individual (or supervisor) for decision
Excellent computer skills, including Outlook, Word, Excel, PowerPoint
Experience with electronic medical records software, ECW preferred
Proficient in office equipment, computer hardware and software programsExcellent oral and written communication skills
